Poor drainage: the number-one cause
Water trapped behind a wall builds hydrostatic pressure, and that is the single most common reason retaining walls fail.

Missing reinforcement
A wall built without reinforcement it needed will bow outward at mid-height, where soil pressure peaks.
Many low residential walls hold fine on their own, but a taller wall, a slope above it, or wet clay behind it needs layers of geogrid tying the soil and wall into one mass.
A rigid, unreinforced wall has no way to flex, so ground movement cracks it. Any wall near or above 4 feet, measured from the footing, should be engineered.
The wrong backfill
Packing native clay against the back of a wall instead of free-draining gravel is a quiet, common cause of failure.
Clay holds water, expands when wet, and presses hard on the wall.
A crushed-stone or gravel zone directly behind the face drains that water away and keeps pressure low. Clay and organic soil belong farther back, never against the wall.
Load it was never built for
Adding weight a wall was not designed to hold, such as a driveway, shed, hot tub, or row of heavy planters near the top, can overload it fast.
Builders size a wall for an expected load, so an addition later changes the math.
Even a parked car or a pool near the edge raises the pressure beyond the design. Tell the designer how you plan to use the space above and below before the wall goes in.
Tree roots and frost
Tree roots push against and under a wall as they grow, and frost lifts a shallow footing as trapped water freezes and expands by about 9 percent.
Roots can shift blocks, lift a footing, and break up drainage. Frost heave tilts a wall that sits above the frost line.
Plant aggressive trees well away from any wall, keep mulch off the back so it does not block the weep holes, and set the footing deep enough to stay below freezing soil.
Warning signs your wall is failing
Catch a failing wall early by watching for leaning, bulging, cracking, and water signs.
The common red flags are a wall that leans or tilts forward, a face that bulges or bows in the middle, horizontal or stair-step cracks near the base, joints that have separated, and a gap opening between the wall and the soil behind it.
Add water seeping or staining through the face, efflorescence, soil washing out at the base, and tilted caps. Any of these after a heavy rain deserves a prompt look.
Can a failing retaining wall be repaired?
Often yes, if you catch it before it topples.
Small cracks can be cleaned out and filled with hydraulic cement, and clogged weep holes can be opened.
A leaning or bulging wall usually needs temporary bracing, then a proper fix at the base: re-excavating to reinstall drainage, widening or deepening the footing, or rebuilding the lower courses. A wall that has already collapsed has to be rebuilt from the base up, so early action saves the most.
